Expression of Interest Form

The expression of interest form is hosted on REDCap — a secure, university-standard research data collection platform. Completing this form takes approximately 5–10 minutes. Your information will be handled confidentially in accordance with the Privacy Act 1988 (Cth) and Western Sydney University's Privacy Policy.

Referred by a healthcare provider?

If a GP, specialist, or allied health professional has suggested you may be eligible for this study, please mention this when you contact the team or complete the form. This helps us track referral pathways for feasibility purposes.

Privacy & confidentiality

Any personal information you provide through the expression of interest form will be collected and stored by Western Sydney University / NICM Health Research Institute for the purposes of administering this clinical trial.

Your information will be handled in accordance with the Privacy Act 1988 (Cth) and the Western Sydney University Privacy Policy. Data will be stored in a secure REDCap environment managed by Western Sydney University.
